The best posture for passing stools is squatting. In this position, waste elimination is easier, faster and complete.


Sitting, on the other hand, obstructs the passage of waste through the colon. Elimination is difficult, requires straining and can never be complete.

Dear Editor,

The author's focus on "management" avoids a much more valuable aspect of medical care, namely the need for prevention. Diverticulitis starts as diverticulosis, whose etiology has baffled the medical profession for over a century. Therefore, by solving the mystery of diverticulosis, we can automatically prevent its costly and painful downstream effects.

For many decades, the fiber theory of Denis Burkitt was accepted as the most plausible explanation for the high prevalence of diverticular disease in western countries. In 2012, Anne F. Peery, the author of the present article, published a study debunking Burkitt's fiber theory, but offered nothing to replace it.

I informed her and her colleagues that Burkitt himself had an alternate theory which offered much more promise. In 1973, the British Medical Journal (precursor to the BMJ) published Burkitt's paper, "Some Diseases Characteristic of Modern Western Civilization." (https://www.ncbi.nlm.nih.gov/pmc/articles/PMC1588096/pdf/brmedj01541-003...).

The paper of course focuses on his fiber theory, but it also mentioned the unnatural method of defecation used in the western world. In this and other publications, Burkitt frequently wrote about the role of toilet posture in many of the ailments of western civilization.

In 2021, Peery published an editiorial, "Bowel Movement Frequency Linked to Increased Diverticulitis Risk in 2 Prospective Cohort Studies." (https://www.cghjournal.org/article/S1542-3565(21)00103-8/fulltext). The link between bowel movements and diverticular disease perfectly supports Burkitt's alternate theory. The western toilet has turned the act of defecation into a self-destructive act, whose consequences include the formation of diverticula.

For more than 20 years, I have been studying the pernicious effects of the western toilet, leading to many diseases that continue to baffle the medical profession. In response to another article by Dr. Peery, I published "Etiology of Diverticulosis," which details the biomechanical consequences of abandoning the natural squatting position. I hope that readers will closely study my analysis and consider doing research to finally solve the mystery of diverticular disease and other ailments that haunt our society.
